First step is to find your well registration number. It is a 55-_________number. I can help with that process if needed. The state will need a parcel number, address and owner name to be able to locate the well. Once the well registration is found:

Second step is to call to see if a Statement of Claimant has been filled on the well.
That number is a 39-___________ number. Property owners will need to call 1-866-246-1414 to see if a prior claim has ever filed a claim. They will be told either “Yes” or “NO”. If the answer is “YES” they need to ask who’s name is on the registered claim. They will have to file a transfer form if there is a prior claim on the well.

If the answer is “NO” a Statement of Claimant has not been filed, they will need to file an original Statement of Claimant, in their name on the well.

Might be wise to verify in what “Watershed” the well is found.

All forms must have notarized signatures of all parties.
